1. Initial/follow up consultation – £250 / £200.
New consultations are booked for 20 minutes. Please arrive promptly. Any imaging suggested/arising from the consultation such as x-rays are payable directly to the hospital not to Mr Craig. You will be informed of any such investigations at the time of the consultation.
2. Payment of invoices are to be made within 30 days or receipt of the clinical letter.
3. Late payments are liable to standard interest charges of 8% .
4. Cancellations with less than 24 hours notice are charged at £100. We are happy to reschedule your appointment before this time frame.
5. If you have private medical insurance, please contact your insurer before your consultation, to check the terms of your policy, particularly the level and type of cover you have, including any reimbursement limits on individual consultation fees. I am recognised by the private medical insurers listed;
AXA, Vitality , AVIVA , BUPA.
Please note that you are responsible for any fees not covered by your insurer.
6. I do not have any financial interests in a hospital or any equipment there.
